SPECIAL AWARDS PROGRAM

An Eye For Order has established the Referral Rewards Program to reward those who refer business to us. To participate in the Program, please register below.

Referral Criteria and Program details are as follows:

• Up to $50 per referral
• Referral must be a new client of which An Eye For Order has never served.
• Referral must purchase a minimum of 3 hours for the member to receive the reward.
• Referral must pay bill in full prior to any rewards being paid to member.
• All member rewards are paid within 30 days of completion of service. Checks will be mailed to the address on the member's registration form. Members have the option to have rewards donated/designated to a third party.
• An Eye For Order reserves the right to decline or discontinue membership at any time without notice.
